Mindfulness-Based Stress Reduction Program
The Mindfulness-Based Stress Reduction Program includes skills training in meditation methods as well as gentle stretches (Mindful Movement/Yoga) exercises. I understand that if for any reason I am unable to or think it is unwise to engage in these techniques and exercises either during the weekly sessions, or at home, I am under no obligation to engage in these techniques nor will I hold the instructor or Working Mindfulness LLC liable for any injury incurred from these exercises.
Iunderstand that this is an educational program and does not constitute and is not a substitute for medical diagnosis, advice or treatment. I understand that while this program may have therapeutic benefits, it is not psychotherapy or a substitute for psychotherapy. I agree that ifl am in need of psychotherapeutic support or intervention, I will seek it through appropriate channels including, but not limited to, asking for referrals from the program facilitators.
I agree to assume all risk for any physical, mental, and/or emotional consequences of participating in this process/program.
I specifically and expressly hold harmless, indemnify and lease all facilitators and teachers of this program from any and all liability for the results of the educational guidance that will be or have been provided. I understand that no guarantee is made as to the outcome of this educational training program.
I give permission for the instructor to contact any health care provider that I have previously named on the pre-program survey, completed prior to attending orientation.
I understand that there are no refunds of payment.
Iunderstand that to the best of my ability; I am expected to attend each of the eight weekly sessions, the daylong session and to practice the home assignments for 40-60 minutes per day for the duration of the MBSR program to the best of my ability.
